Diet soda. Zero-calorie energy drinks. That “sugar-free” yogurt you grab at the grocery store.
They feel like the smart choice. Fewer calories. No blood sugar spike. But a new study suggests there might be a hidden tax on your brain.
Research published in Neurology in September 2025 found a worrying link between artificial sweetener consumption and accelerated cognitive decline. Specifically, it affects memory and thinking skills. And the risk isn’t distributed evenly. It hits harder in adults under 60 and those with diabetes.
Which artificial sweeteners are linked to brain aging?
The study looked at seven common substitutes: erythritol, aspartame, saccharin, acesulfame-K, sorbitxitol, xylitol, and tagatose.
Researchers tracked 12,772 adults over eight years. The median age was 52. Participants self-reported their diets, listing everything from flavored water to low-calorie desserts.
The results were stark.
People who consumed the highest amounts of these sugar substitutes saw their cognitive skills decline 62% faster than those who consumed the least. That gap translates to roughly 1.5 years of extra brain aging.
The middle group didn’t fare much better. They experienced a 35% faster decline, equivalent to about 1 year and 3 months of accelerated aging.
Why is the link so strong for younger adults?

The connection was weakest in people over 60. Among those younger than 60, declines in verbal fluency and overall cognition were significantly steeper.
Why do artificial sweeteners potentially harm cognition?
Causation hasn’t been proven. The study was observational. But researchers have theories.
Claudia Suemoto, lead author of the study, points to neuroinflammation. Some artificial sweeteners may trigger inflammatory responses in the brain. This is a known early process in Alzheimer’s disease and vascular cognitive impairment.
Another theory involves the gut. Disruption of gut microbiota could affect brain health via the gut-brain axis. It might even compromise the integrity of the blood-brain barrier.
Not everyone agrees on the mechanism. Clifford Segil, a neurologist in Santa Monica, doubts neuroinflammation is the primary driver. He argues it comes down to energy.
The brain runs on glucose. When you feed it a substitute, the metabolic response changes.
“A substitute sugar that is not the brain’s natural sugars can affect the brain,” Segil says. “Energy dressed up as a sugar substitute does not work the same in your brain as actual sugar.”
How reliable is this study?
Take it with a grain of salt. Or maybe a packet of Stevia.
The data relied on self-reported diet information collected just once, eight years ago. People forget what they ate. Product formulations change. The International Sweeteners Association criticized the study for this single-point-in-time data collection. They argue it introduces bias.
They also note that not all sweeteners acted the same way. Tagatose showed no link to cognitive decline in this cohort. Whether that means it’s safer, or simply less commonly consumed by the participants, remains unclear.
Global health authorities still maintain that low-calorie sweeteners are safe.
Should you stop drinking diet soda?
Panic isn’t the answer. Moderation is.
Keri Gans, a New York City nutritionist, frames it as a moment for balance. One diet soda a day? Don’t lose sleep. But binging on protein bars packed with sugar alcohols while on GLP-1 medication? That’s where the cumulative intake gets dangerous.
Many people don’t even realize they’re consuming these sweeteners. They hide under the label “sugar alcohols” in snacks and supplements.
For diabetics, the dilemma is tougher. Doctors often recommend these substitutes to manage blood sugar. Gans advises individualizing choices. Reduce overall sweetness when possible. Don’t just swap sugar for sweetener in unlimited quantities.
The bottom line? More research is needed. We need to know if alternatives like apple sauce, honey, or coconut sugar offer a safer path. Until then, your brain might be paying for every zero-calorie sip.
